Use this checklist to gather the information you will need to apply.

Apply through a Navigator.

Open enrollment in the Health Insurance Marketplace ends March 31, 2014.

If you are confused by the options or are having trouble signing up help is available. A company called Cognosante has been contracted by the US Department of Health and Human Services to provide certified "Marketplace In-Person Assisters" to provide impartial information about your eligibility and the Qualified Health Plans from which you can choose.

These "Assisters" will be available at the Main Library February 4 and February 11 from 1:30-8:00 p.m. to answer your questions and help you enroll on the spot if that's what you determine to do. The service is completely free. If you are thinking of enrolling and would like help completing the process please bring the following information when you come to Main Library:
  Social Security Number (or document number for legal immigrants)
  Birth Dates
  Employer and income information for every household member who needs coverage (pay stub or W2)
  Policy numbers for any health plans in which you are already enrolled
  Information about the health coverage available through your employer (if any)
  Valid email address to create an online account

Walk-ins are welcome but if you would like to make an appointment for a specific time slot please contact Kathy Freisinger at 973 948-3660 ext. 3401, Monday-Friday between 9:00-1:00

Help is available 24/7 and in 150 different languages.

Receive in-person help from an officially designated Navigator. The closest Navigator for Sussex County is Zufall Health Center. They can be reached at 973-328-3344 or 908-452-5366.
